Information for registration

Congress registration (active, passive - individual or group, company) is only possible using these web pages. Please, click on "REGISTRATION" in the congress Menu. 

The registration fee includes the admission to professional meetings, the congress bag with the Program and other materials, a voucher for the CD-ROM with congress abstracts, two vouchers for lunch packs (May 17 and 18), free admission to Gala opening of the congress with a welcome drink.
The registration for exhibitors that is free of charge only entitles the exhibitors to enter hall A1 and professional meetings; partial registration only entitles the exhibitors to enter hall A1 + the lunch pack.

The acceptance of fees via bank transfer FINISHES by payments sent on 7/5/2010 at latest; later the payment is unidentifiable and the participant can be asked to pay again on the spot, where the registration fee is higher.

Active participants (1st authors of presentations) will pay the same registration fee as the other participants. CSC members aged below 35 (1st authors) can ask the Galena Endowment Fund (Healthy Heart) - www.nadace-galena.cz for congress fee refund.

Registration fee CANNOT BE PAID BACK!!!!! (in case of double payments, failure to arrive etc.).

It is possible for group registrations that each of the group members pays the fee individually. All members of the group will have their special numbers for the registration and if the payment is not made collectively, this number is to be used as the variable symbol. If the payment comes to the account with the group variable symbol in the non-corresponding total amount, it will not be possible to match it with the individual registered members in the group.

Please, pay the registration fee to the account of Czech Society of Cardiology number 502-464517024/2700, the variable symbol is your registration number included in the e-mail “Confirmation of the registration to the congress”. Pay the registration fee only after the on-line registration.

KEEP THE CONFIRMATION OF THE REGISTRATION FEE PAYMENT (PAYMENT IN ADVANCE) AND BE READY TO PRESENT IT WHEN REGISTERING.

The on-line registration (registration in advance) will be finished on April 23, 2010; later, only the registration on the spot is possible.

Name tags
Each congress participant, including exhibitors, will obtain a name tag which authorizes him or her to enter the congress spaces.

Colours of name tags: 
Red                    registration for the entire congress  
Green                 one-day registration (the specific day being marked on the tag) 
Golden               society committee, congress guests, CSC honourable members
Blue                    exhibitors
Yellow                health care workers
Purple                organizational committee (the members will be also wearing coloured T-shirts with congress logo for better identification)
 
All entrances to the congress spaces will be guarded by a security agency so make sure you are wearing the name tag on a visible place.

IT IS NOT POSSIBLE TO ENTER THE SPACES WITHOUT THE NAME TAG – if you lose it a copy will be made for 200 Czk.
