LONG TERM HEALTH OUTCOMES OF PATIENTS AFTER THE END OF DUAL ANTICLOTTING THERAPY USED IN PATIENTS WITH IMPLANTED DRUG-ELUTING STENT. (HOMEDES REGISTRY, HEALTH OUTCOMES AND MORTALITY EVALUATIONS AFTER INVASIVE CORONARY TREATMENT USING DRUG ELUTING STENTS)

Aim: The aim of the observation was to find out the occurence of late (more than 6 months) clinical events caused by late thrombosis of drug eluting stent (DES) in patients after finishing the dual anticlotting therapy.
Methods: Total 273 patients have been underwent PCI with the implantation of DES in the period from Janury 2004 to May 2006 and 171 patients of this cohort have finished the 12 and 18 month study observation. Coronary stent thrombosis has been defined: 1) the angiographic demonstration of a thrombus; 2) repeated infarction in the place of the original intervention on the same coronary vessel; 3) sudden cardiac death of the observed patient.
Results: We noted 9 cases (5.2%) altogether of the late coronary stent thrombosis in 18 months study visit after implanted DES. Sudden cardiac death clinically demonstrated 4 cases and non-fatal myocardial infarctions in the target vessel area achieved 5 cases. The best part of clinically cases happened in 7-12 study window with the average 121 days occurrence after clopidogrel discontinuation. We noted 7% cases with the repeated target vessel intervention on the same artery. The incidence of the cardiovascular events was 12.2%.
Conclusions:  The incidence of this cardiovascular events correlates with the recent findings of BASKET-LATE study.
Our observation refers to the trend in the higher incidence of the late cardiovascular death and late non-fatal myocardial infarction after finishing the dual anticlotting therapy in patients with implanted drug-eluting stent.
